Author Interview / Profile questionnaire
For your thoughtful consideration:
Please answer as many questions within your comfort level / range. Questions are in no particular order. You may want to answer and rearrange your selected questions in the order you would prefer to see on your interview page. Just skip over any questions that don’t apply and some of the questions are somewhat redundant. Also, you may include additional questions and answers of your own creation. We want your interview page to reflect your profile and personality. In addition, this is an opportunity to convey any desired message(s) to your audience. We will include any website information that may be available. With your permission, information from online sources could be used in creating the author profile. Also, rather than use the question / answer format listed below, you are welcome to submit your own free-form profile.
